Fab Form was founded in 1977 as a fabrication shop with the goal of serving the greater Cleveland, Ohio area with quick turn, high quality weldments and fabrications at a competitive price. Customer demand proved strong and soon after starting, Fab Form expanded to include short run metal stampings. Metal stampings became the core competency and growth engine over the next 25 years. Our current capabilities include metal stamping, welding and fabricating, machining, laser cutting, assembly and finishing.

Fab Form, over its thirty year history, has grown from a small shop with one employee to its current size of 35,000 square feet and 40 employees in Mentor, Ohio. We service industries such as Medical Transportation, General Industrial, Computer Controls and many more across the U.S and Canada.
